Improving The Patient Experience: Part 2 – Checking Your Blood Pressure

Hello everyone!  This is part 2 of a series around how we work towards improving the patient experience!  Part 1 related to Covid-19 and Flu vaccinations, and here we are going to discuss hypertension and the importance of checking your blood pressure!

Checking Your Blood Pressure

The U.S. Preventitive Task Force gave their current recommendation statement on April 27, 2021 concerning hypertension.  The USPSTF recommends screening for hypertension (high blood pressure) for adults 18 years or older with office blood pressure measurements.  They also recommend obtaining blood pressure measurements outside the clinical setting for confirmation before starting treatment.

So what does that mean for you?

Well everyone knows that high blood pressure can cause major problems  These can range from the head (early dementia and bleeding strokes) all the way to the toes causing peripheral artery disease and amputations, and all organs in between.  Many studies have shown that lowering blood pressure to acceptable levels (140/90 and preferably 130/80) can prevent these problems.  We have many new medications that can help lower blood pressure but what a lot of people don’t recognize is that non-medical remedies can be just as effective such as diet, exercise and better sleep.

  • Diet: Here at South Macomb Internal Medicine we focus pretty heavily on lifestyle choices that help impact these things, especially when it comes to nutrition for example with many posts here on this blog by Nanette Cameron, RDN.
  • Exercise: We can also refer to physical therapists / trainers for proper exercise regimens.  Generally we recommend some form of aerobic activity for 30 minutes at least 5 days per week.
  • Sleep: We also analyze sleep disorders on a regular basis and if necessary can refer to a sleep lab or perform in-home sleep testing to make a diagnosis.  Often we are told of patients falling asleep while receiving their instructions at home when they first put on the CPAP mask because they are so sleep deprived – NOT because they are bored I assure you.  Ha!

Of course we offer the newest in medications and often we can control blood pressure with one pill per day, but what’s old is new, and one of the oldest medications available is spirnolactone which has seen a resurgence as this diuretic has been shown to lower blood pressure especially in difficult to treat cases.

Hello everyone!  I hope you are doing well, and we are introducing a series around how we work towards improving the patient experience!  A little background on this and some highlights of the initiative as outlined by Blue Cross Blue Shield of Michigan:

Covid-19 and Flu Vaccines

We know that our entire team can affect the health of patients and how they assess their health care experience, so we are going to discuss a number of points of emphasis starting with Covid-19 and Flu vaccinations!

First let’s talk about boosters.  The newest data coming out of Israel (which is administering the most boosters to date) is that they are very effective and safe.  We still are not sure if they are necessary because we haven’t seen a big increase in cases among vaccinated individuals.  We are happy to discuss your options with you.

As for Flu – the high dose is in and available and they are first come first served for those aged 65 and over.  We expect an increase in flu this year since people aren’t quite as socially distanced as they were at the height of the pandemic.

These are the types of very real questions that we know many of you have based on our interactions with you at our office.  We are doing our very best to keep up with the up to the minute data so that we can provide the best experience for our patients!  Check back for part 2 which will focus on hypertension!

Some Current Thoughts On Covid-19 – Delta and Where We Are Now

We have had plenty to say here on this blog about Covid-19.  We’ve covered plenty of angles from encouraging people to get vaccines, to remembering not to neglect your “other” (non-covid related) health, to myth-busting and more, and I wanted to add some current thoughts.  As we have seen this continues to be a fluid situation with the Delta variant now being the dominant strain and causing serious problems around our country.  There are still too many people who have not been vaccinated and the CDC is set to approve booster shots soon for those who have.

So what do you do?

Current Thoughts

Well, for starters we still can’t emphasize enough how important it is to get the vaccine if you have held out up until now.  We still feel strongly about this.  The vaccines are indeed safe and effective.  But beyond that we also say maintain a cool head and get back to the basics that we were using before there was a vaccine – washing your hands, using common sense with social distancing, using a mask indoors perhaps – just make good choices while we’re in this surge.

We also want you to read what we feel is an outstanding article published this week on KevinMD.com.  It goes into some detail about the Delta Variant and the vaccine, but there is no slant here.  No politics – this isn’t a big media source – just a doctor writing it, in fact a psychiatrist with some really interesting insights on this.  We encourage you to digest what is said here and share it with whomever needs to hear it from any standpoint.  That includes people who follow the science and those who don’t trust it.  Democrats, Republicans, anti-vaxxers & vaxxers, anti-maskers and maskers – anyone.  Let’s talk it out.  Let’s help each other.

Let’s Talk About White Coat Syndrome – For Real This Time

We recently posted an article on our Facebook page from themuse.com entitled 10 Reasons You Should Go To The Doctor (Even When You’re Healthy).  It’s a bit of a recurring theme we’ve returned to in the context of a COVID-19 pandemic that doesn’t seem to want to completely go away (GET VACCINATED if you haven’t yet – yes it’s safe).  We don’t want people to neglect any other health issues they may have that went untreated while they were home avoiding the virus for a year or so and Dr. Goldberg wrote about that too.  We want people to take care of their health and see their doctor even when they are healthy as the article states.

But maybe it’s not that easy for everyone.

We’ve written about White Coat Syndrome before on this blog but the one time we made it a featured topic it was all in good fun with a humorous take on why you might have it.  However we do acknowledge (as we did in the humorous post) that it’s certainly real for many and we want to explore that in a little more detail.

In our humorous post on the topic we included a link to this article from Medical News Today that goes into great detail about White Coat Syndrome – what it is, diagnoses, and even prevention, but that article is a little light on the prevention part.  We would like to point you to another one very thoughtfully written by a family practice in South Carolina called Thrive Family Medicine about how to overcome it with several actionable tips.  We hope this helps!  The last thing we would ever want to see is one of you avoiding a check up because of your anxiety over visiting the doctor’s office.

5 Reasons To Get Excited About The Future Of Medicine

Advancements in medicine have made our lives better on Earth in so many ways over the centuries, and as with all technology it seems to be picking up pace.  Just look at how fast the Covid-19 vaccines were safely developed!  Please get yours by the way!

Meanwhile there are myriads of reasons to be optimistic and excited about new developments but here are a few articles that have caught our eye recently that we wanted to share in one place here on the blog.

5 Reasons To Get Excited About The Future Of Medicine

  1. The fight against Alzheimer’s Disease seems to be progressing.  According to this article Alzheimer’s is the third leading cause of death in non-pandemic years of older adults, surpassed only by heart disease and cancer.  It’s a horrible disease and new hope for people dealing with it is exciting indeed.
  2. The future of specialized and precision medicine means better treatment.  In the future therapies will be increasingly directed based on an individual’s profile. Precision medicine has been particularly successful in targeting various aspects of DNA in the treatment of various diseases, particularly cancer.  DNA repair and other advancements are really encouraging as outlined in this article!
  3. Wearable technology is advancing to the point of providing valuable data to help monitor our health.  Holy moly – it’s amazing where some of this is going.  The latest Apple watches for example are likely to gain blood pressure, blood glucose and blood alcohol monitoring.  It’s really exciting!
  4. The ocean – yes the ocean – is becoming a resource for new drugs to fight cancer.  How neat is this?  As the final quote in this article tells us, “You might think that we know the depths of the ocean and the biodiversity of the ocean, but it is still largely uncharted.  There is really cool new chemistry in the ocean and we just need to be brave to go in and find it.”
  5. MANY MORE – okay I’m going to cheat a little here but this article from the Cleveland Clinic outlines 10 more – some of which are really incredible like breakthroughs in multiple sclerosis and cystic fibrosis treatments!

Thoughts On Building A Community, Not Just A Practice

Practicing medicine is a very serious endeavor.  A heck of a lot of training goes into becoming a doctor.  Pre-med alone is tough – organic chemistry anyone?  Not to mention getting into and surviving med school, and then there are residencies, board exams – it’s high pressure and a challenging curriculum.  When you come out the other side and earn the title of “Doctor” you’ve been thoroughly tested and trained.  It’s a long hard road.

I’m not complaining – just providing context.

Community

 

Meanwhile a whole lot of effort and hard work goes into building a practice.  Learning to become a doctor is one thing.  Starting a business is a whole other set of skills and challenges and plenty of risk.  It takes time and running a business as many of you probably know is not easy.  And even when you’re “established” there are many variables out of your control such as…..oh I don’t know……a pandemic perhaps?  Granted everyone has been challenged by that, but at the core of the pandemic is the virus itself and people’s health which as doctors puts us at the heart of the matter.

Again – not complaining.  It’s a gift to be in the profession of helping people and being able to do it for a living!

But here’s where we get to the nitty gritty of those two things and taking them to the next level which is my point:

If it takes perseverance, smarts and motivation to become a doctor, and savvy and solid decision making to run a business, it takes humanity to build a COMMUNITY and that’s what we are trying to do here!  Sure we want to be profitable and provide proper care, but medicine can be heavy.  There’s laughter and tears, pain and relief, anxiety and joy, and plenty in between, and embracing them all while building relationships is what makes a community.  A community around our practice.

We have enjoyed this new path of social media – Facebook, our newsletter, blogging here on the website and the like.  Sharing Saturday Silliness, Foodie Fridays, Tuesday Trivia and plenty more on these different platforms allows us to inform, teach, laugh and learn together and we love getting the feedback from all of you – our patients!  We like interacting with you and knowing you – not just treating you, and that goes for our entire staff!  We hope you are enjoying the content and here’s to better times ahead and the continued journey!
